I grant permission.

I grant permission to The Daniel Mucatel School PS 115 Brooklyn, hereinafter known as “PS 115” the right to photograph, videotape, interview and/or otherwise record my child’s likeness, voice, performance, poses, actions, stories, and appearances, and to use such media and recordings in whole or in part, or in combination with other media or recordings for the terms and conditions below. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.

CONDITIONS(Required)

I authorize PS 115 as well as any of its representatives and employees the right to take photographs and/or video of the subjects identified above. I authorize PS 115 to copyright, use, and publish the same in any means deemed necessary including but not limited to print, electronic, or transferring of image. I agree that PS 115 has permission to display such photographs and/or video of the subject with or without my name and for any lawful purpose, including but not limited to such purposes as publicity, illustration, advertising, and web content.

ACCEPTANCE(Required)

I hereby waive any right to review, approve, format, and/or style the finished media or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image. I understand that such media is exclusive property of PS 115. I hereby release and discharge the parties, as well as any and all of its their employees, agents, and/or agencies acting on behalf of the parties, from any and all liability, claims, demands, and/or causes of action that I may hereafter have for injuries or damages of any kind arising out of my participation.
